I've got a footswitch from a m-basis 2000. Same thing only it's labeled differently and it doesn't have the connectors to mount on the back of a mark iv.
one of the LED's doesn't work or is really dim (can't remember). I used it with one of my mark iv's before I sold it.

Keep your eyes on eBay for one... I just picked up a Rectoverb footswitch for $42.50 shipped on there!
