Rectoverb 50 V1 Preamp Tube Trouble?

Has anyone had trouble w/ their V1 Preamp tube quit working, even after replacing it twice? Maybe it's just a bad run of 12AX7's I have, but just replaced it
2 x's in less than 10 minutes. Any advice would be appreciated. Thanks!

Rectoverb is a post-'91 amp, I'd repost this in the 'Modern Era' forum if you want a quicker answer
