Anyone have one that they can scan for me? Better yet, all I really need to know is, V5 is the phase Inverter and is a 12AX7 right? And V4 is for the reverb and is a 12AT7?

Boogietone, you're right. Here's the complete breakdown:
V1A First gain stage
V1B Post tone stack recovery
V2A Third gain stage for R1 and R2, or fifth stage for Lead
V2B Effects return
V3A Third gain stage for Lead
V3B Fourth gain stage for Lead
V4 Reverb
V5 Phase inverter

12AT7 for V4, and 12AX7 for the rest.

MesaDCLP, the official Mark IV thread is sticky already, but my thread is buried somewhere in page 29 or so, pretty difficult to find yeah.
