Looking for settings sheet

I'm not sure what this is actually called, but it was on top of my Mark V 90 head. It's a sheet with demo settings and descriptions. What are they called and are they available for purchase?

Not sure if these are the same but on page 7 of the manual there are sample settings and more starting on page 45.

No. Its a hard sheet that's lays on top of the head/combo. I have a pic, but can't attach here.

Called Mesa. According to them they quit including those some time ago and do not have anymore. So, if I'm going to get one someone would have to sell me theirs. Anyone?
