How do I get a Mark V setting card / coaster

Does anyone know how can I get a Mark V setting card / coaster?

I'm not sure that they come with the settings card nowadays.

You mean the card on top of the amps in the store?

If you only want it for the settings I'm pretty sure it's the same examples that are shown in the manual.

Those cards were available only with the first few MK5's, and have not been available for quite some time. Thanks!

I got one of the Mark V's from the first shipment to the Hollywood store and I did not get a card. The box was sealed when I took possession of it.
