    Bit of a necro bump but this is what I have working, pin 1 is supply 12v, pin 2 channel switch, pin 3 eq, pin 4 not used, pin 5 12v shield is ov I have 2 and 3 and shield connected to a trs plug and use a gizmo for channel switching, there is a cable available for amp gizmo but not in the UK

    out of interest have you tried the PI tube for a known good one, it would also be worth checking if you have a nice glow from the output tubes, chances are its not serious, usually boogies are pretty much bullet proof so i would suspect its tube related

    try bypassing the effects loop with a patch lead, my dc5 had a similar problem wihich this cured, is so replace the effects loop jacks or give them a good clean, apart from that PM Monsta-Tone, chances are he can point you in the right direction
