Remove all preamp tube shields (if applicable) and tap the tubes gently with a pencil's eraser. That should identify the bad tube.

Loop switch is wired for parallel or serial and works either way. Real nice addition but will most likely use in series as my other amps are and only my TC Electronics have dry kill option. Dialed in great tone in Vintage mode and loving it.
